Abstract   In patients with primary extragonadal germ cell tumor, treatment with combination chemotherapy followed by surgical resection of residual tumor is usually regarded as standard therapy, but the best treatment for such patients still remains a problem. Between November 1987 and October 1989, 8 patients with extragonadal germ cell tumor (7 mediastinal and 1 retroperitoneal) were treated with bleomycin, etoposide and cisplatin (BEP). Four (50%) are alive, 2(25%) with no evidence of disease and overall survuval time was 3.7-30 months (median 9.9 months). Toxicity of chemotherapy was relatively severe. In 71% of the cycles leukopenia was developed and all patients experienced nausea, vomiting and alopecia. Neurotoxicity was observed in 25% of patients and pulmonary toxicity in 50%. We conclude that the BEP regimen has relatively good activity in patients with extragonadal germ ceil tumor, but advanced disease at presentation contributes to the poorer prognosis for these patients. Earlier diagnosis and appropriate surgical resection may improve the long-term survival of patients with this disease.
